PGM Dress Form Rental Agreement
RENTAL PRICE: All Dress Form rental pricing is according to the size and value of the dress form.
·        Half body forms - $100.00 (1 week)
·        Large Half body forms - $120 (1 week)
·        Full Body Forms- $150.00 (1 week)
·        Large Full Body Forms - $180 (1 week)
DEPOSIT FOR EACH RENTAL DRESS FORM:   is the full amount of the rental size dress form listed price on PGM website. 
4 weeks is the maximum limit for rentals of any dress forms.  After 4 weeks end,  the deposit will be charged as the purchase of this size rental dress form(the deposit will not be refunded anymore, and this rental dress form no need to be returned).
The Deposit will be refunded when the rental dress form is returned in time with no damages.
DAMAGES:  such as stains, dirty, marks, rips, missing or loose pieces or break the major parts etc.,  if the rental dress form with damages when returning, the deposit will not be refunded but to be charged as the purchase of this damaged rental dress form. If no damages on the rental dress form when returning in time, the deposit will be full refunded.
EXTEND RENTAL FEES: If the rental dress form is not returned back in time, the extend rental price will be charged accordingly(4 weeks is the maximum rental limit).
When you sign this rental agreement, means you understand and accept the rental terms the above and take the liability for the dress forms while in your possession.
